Agreement to receive information about Creative Labs’ activities and offers

I agree to receive information regarding Creative Labs’ activities and offer to the e-mail address provided by me to Creative Labs Sp. z o.o. with its registered office in Kraków at ul. Dworska 1, 30-314 Kraków. The consent is voluntary. Consent may be revoked at any time.

Pursuant to Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (“GDPR”) I have been informed that:

1.Controller of personal data

The Controller of personal data is Creative Labs Sp. z o.o. with its registered office in Krakow at ul. Dworska 1, 30-314 Kraków, entered in the register of entrepreneurs of the National Court Register, kept by the District Court for Kraków-Śródmieście in Kraków, XI Commercial Department of the National Court Register under the KRS number: 0000774253, NIP: 6762562484, share capital: PLN 5,000.00, e-mail: office@wearecreativelabs.com (Hereinafter reffered to as: the “Controller”).

2. Objectives and grounds for processing personal data

The Controller processes your personal data in order to inform you about its activities and offer (direct marketing) and to send you a free e-book containing information about the Creative Labs methodology, which is a legitimate interest of the Controller – pursuant to Article 6(1)(f) of the GDPR.

3. Data recipients

The Controller may make your personal data available to its subcontractors (entities it uses for the processing) such as:

1. IT service providers (e.g. hosting providers and email senders);
2. entities providing IT support services.

In order to process your personal data the Controller may use the services of entities which are based in the USA. Therefore, personal data may be transferred outside the European Economic Area. The Controller declares that in this case it will only use the services of entities that belong to the Privacy Shield programme and provide an adequate level of data protection.

4. Data retention time

The Controller stores the data obtained in order to inform about its activities and offer (direct marketing) until the data subject object to their processing for this purpose or until the Controller decides to delete it.

The data obtained in order to submit a free e-book containing information on the Creative Labs methodology is stored by the Controller for 1 year.

5. Rights of the data subjects

The data subjects have following rights:

1. the right of access to personal data and the right to receive a copy;
2. rectification of personal data;
3. right to erase the personal data;
4. right to limit the processing of personal data;
5. right to transfer of personal data; 
6. right to object to the processing of personal data, 
7. right to complain to the supervisory authority.

In order to exercise the above rights, you may contact the Controller.

6. Information on whether data are voluntary or necessary

The provision of data by you is voluntary. 

If you do not provide the data for the purpose of processing them to inform about the Controller’s activities and its offer, you will not be able to receive the aforementioned information.

If you do not submit data for processing for the purpose of sending a free ebook containing information on the Creative Labs methodology, you will not be able to receive this ebook.
